• Small filling - (1 surface): This is a small cavity on a tooth (like a dot) - decay is on a single surface
• Medium filling - (1 - 2 surfaces): More deterioration to the tooth (the cavity is either single surface but very deep or decay propagates on 2 x tooth surfaces). If your filling qualifies for a medium size filling additional $50 payable direct to the merchant will apply per medium filling
• Large filling - (3 surfaces or more): This is a seriously decayed tooth that requires restoration. If your filling qualifies as a large filling additional $89 per large filling will be payable to the merchant
• Heavy buildups and/or deep gum treatment for clean & polish will incur an additional fee - dependent on the amount of the severity
