Deposit for One Premium Osstem® guided Dental Implant surgery including Dental Consultation & Procedure with Abutment and Crown. Online bookings essential for initial appointment. Four appointments minimum required; The order of appointments is as follows: 1) Consultations - before and after 3D scan, includes preliminary impressions and local xray. 2) Implant surgery. 3) Impression for crown. 4) Crown (and abutment) placement in mouth. Consultation, impression, and placement appointments can take place any time during opening hours, however implant surgery to take place on Wednesday and Thursdays exclusively. A 24-hour cancellation policy applies for all appointments, a charge of $100 payable direct to merchant will be required in the event of cancellations within this period or no-show appointments. Voucher must be used by one person. Any additional treatments will be at an extra cost & discussed with you first. Tooth extraction can be done for you, discussed at time of consultation, case by case. If suitable, merchant will quote directly and customer to pay merchant for tooth extraction. At time of implant surgery appointment, tooth must already be extracted and gum be healed. In the event customer or Dentist finds that the surgery is not suitable and cannot take place, GrabOne will provide a full refund however customer will be required to pay Dentist a consultation fee of $150. For Dentist to confirm suitability for dental implant, customer WILL be required to have a 3D scan. Dentist will advise this at time of consultation and make a referral to third-party imaging company. 3D scans are not included in this treatment or treatment cost, and will have an additional cost from the customer to the third party imaging company, (approx. $250-300). Deposit option requires a further instalment of $1,950 prior to the day of implant surgery and the remaining balance of $1,100 prior to crown/abutment delivery. No cashback.
